By filling out the form, you consent to in-person treatment with us at the 2024 FrostFit Games and receiving helpful emails moving forward!

By signing this waiver, I acknowledge that I am knowingly agreeing to participate in the free injury screen and/or movement assessment.

I understand that choosing to participate in an injury screen or movement assessment means that I am allowing The Resilient Athlete Performance PT LLC to perform screening that may include but not limited to manual therapy, exercise or other treatments.
